KCHC is Your Patient Centered Medical Home
What do we mean by “Patient Centered”? We mean our practice is built around you and your needs. Picture yourself and your provider as the hub of a wheel, in partnership to maximize your health and well-being. The spokes, then, are any specialists, your hospital, your insurance company, your pharmacy, and all your other healthcare and medical needs. Our aim is to work as partners with our patients to provide patient centered services that are comprehensive, timely, convenient, safe, and satisfying.
At KCHC, we want to make sure you get the care you need, when you need it.
The more you understand your health and medical needs, and the more we understand you, the sooner we can make sure you’re on the right path to good health. Please be honest with your KCHC Provider about your medical and health information. Let us know how you feel and what you think. If you are worried about a treatment, if you are using complementary treatments, or if there is anything that has the potential to affect your health, please let your KCHC Provider know that! By working together, we will empower you to be an active partner in your own healthcare.
During your appointments, our providers and staff are happy to answer all your questions so you fully understand your current health, your options and your plan of care. In addition, your KCHC Provider also needs to know if you have any concerns about specialty care or other services you receive outside of KCHC. Your KCHC Provider wants to understand you as a whole person.
Coordination of Care
For those with a chronic condition, complex medical needs or a multitude of medical issues, managing your health can become complicated and frustrating. Many of our patients are under the care of a number of specialists, taking a multitude of medications, and undergoing multiple medical therapies. We can help you sort it out. Let us know if you have questions or if you’re having any difficulties with any of your healthcare.
As your medical home, KCHC will work with you to ensure your healthcare is coordinated among specialists, hospitals, and any other healthcare agencies. Please make sure that reports from your visits to other doctors or facilities are sent to your Primary Care Provider here at KCHC.
KCHC strives to make medical care accessible through minimizing wait times, enhanced office hours, and after-hours access to your KCHC healthcare team through alternative methods such as MyChart, our secure, online communication system. Sign up for MyChart and you can immediately communicate with your KCHC Provider regarding nonurgent medical matters, access your test results, request prescription refills, or manage your KCHC medical appointments.
