What is Care Management?

Care Management is a Medicare program that gives you access to remote care in between your regular office visits. The program’s goal is to keep you healthy, offer you assistance, and prevent any conditions you have from worsening.

A Care Management program reduces unnecessary hospital visits, ER visits, and overall healthcare costs.

Care Management benefits include

  • A 24/7 care line where you can reach clinicians and nurses
  • A customized care plan and care goals
  • Assistance with setting appointments and filling prescriptions
  • Connections to community resources, like transportation assistance

Is this right for you?

You may be eligible for Care Management if you:

  • Have Medicare
  • Have two or more long-term health conditions, such as diabetes, high blood pressure, heart disease, arthritis, COPD, or depression
  • Want extra support managing your medications, appointments, or care plan
  • Would like more regular contact between office visits

How to enroll

If you’re eligible, a program representative will call to ask if you would like to join the program. You can also ask your provider for more information about Care Management and whether you may be eligible.

To enroll or ask questions, call ChartSpan at (855) 242-7877.

Enrollment is voluntary, and you can leave the program at any time.

Important note: If you sign up, you may have a monthly co-insurance payment, and deductibles do apply, even in months you choose not to engage.
